During the verification of your Mollie account, we may request additional documents if further information is needed to verify and confirm your company’s ownership and structure. This may happen when extra details are required to accurately establish the shareholding and organizational setup. In such cases, we might ask you to provide one or more of the following documents:
- A shareholders register, or;
- Rejestr certyfikatów, lub;
- Akt powierniczy (w przypadku, gdy powierzenie jest częścią struktury), lub;
- Oświadczenie UBO od osób trzecich (np. list AML wydany przez regulowanego administratora funduszy, jeśli fundusz jest częścią struktury), lub;
- Raporty roczne (o ile dostępne i odzwierciedlające informacje o UBO), lub;
- An organisational chart leading to the UBO (natural person)and signed by an external party, such as an accountant or legal counsel, or;
- A UBO declaration declaring name and percentage holding of each UBO signed by an external party, such as an accountant or legal counsel. Click on the link to download this declaration form.
